I adore this episode. This is such a beautiful distillation of the Doctor, seeing how they react to situations so similar to the real world. I'm unsure how intentional this was, but I also think this is a wonderful exploration of the Doctor neurodivergence (compared to other episodes like the scene in Under the Lake). Seeing him struggle to understand why he can't help solve the problems of the story when he knows how useful he could be.
I'll now be talking about Tania's scene at the end of the episode: I love how this episode handles Tania's coming out scene and Liv's subsequent response. Previously we've seen Liv have a nonchalant attitude towards disabled people, such as Martin in A Life in the Day, I love how this is extended to Tania's gender. Tania has presented Liv with a fact that may scare some people off and Liv diffuses the situation with such indifference and care. I loved this scene when I first listened to it, this is peak trans representation.
